Filing Your Insurance

Hopp Physical Therapy understands that sometimes the most confusing part of your treatment is sorting out the billing and insurance details. Tammy, our Office Manager, is dedicated to ensuring paying for your services is as convenient as possible. Please contact Hopp Physical Therapy with any questions or requests.

  • Hopp Therapy will submit your services directly to your insurance company.

  • Hopp Therapy will work with you and your insurance company to receive payment for services.

  • Deductibles, coinsurance and co-pays are due at the time of services.

  • Hopp Therapy will file third party claims (auto, personal injury).


Insurance Benefits

Prior to your first visit, it is a good idea to call the Customer Service Number, usually located on the back of your insurance card, to ask about your out-patient physical therapy benefits. Pertinent information includes; deductible, coinsurance and or copayment amount, number of visits allowed in a calendar year, and whether a Dr.’s referral is needed prior to treatment.

If you are unable to obtain information about your insurance benefits, Hopp Physical Therapy will obtain it for you.

  • Tricare (except for Tricare Prime)

  • United Healthcare
